Registration Procedures for projects in Grades
1-5: Team |

Step
4: What
category is your project? Click on the link below for a list of
the categories. You will need to decide on a category before
continuing to Step 5.

Step
5:
Do you
want to fill out the Student Information-Team Form (W1A) on the
computer (preferred) or do you want print out the form and fill
it in by hand? One form is required for each team project. If
you fill out the form online, please print out a copy before you
close the online form window. Press one of following buttons.

6: Click
the following link to download the pdf version of the
Parent (Guardian) Signature Form (W2). One form
is needed for
each team member in the team (i.e., 2 person teams requires 2
forms and 3 person teams require 3 forms).
Each team member
is responsible for completely fill out this form and having
their parent or guardian sign the form.

Each team member’s form must be signed by their parent or
guardian and mailed with the team’s registration package to
participate in WSSEF .

7:
Click the following link to download the pdf version of the
Adult Sponsor Checklist for Grades 1 - 6 Form (W3). Completely
fill out this form and have your adult sponsor sign this form.
The Adult Sponsor may be a teacher, parent, university
professor, or scientist in whose lab the student is working. The
Adult Sponsor is responsible for working with the student to
evaluate any possible risks and to ensure the health and safety
of the student conducting the research and any humans or animals
subjects involved in the project.

Note:
Be sure to attach a copy of the Adult Sponsor Checklist for
Grades 1-6 (Form W3) when you mail your registration package.

8: Did you
complete the three forms from Steps 5 through 7?
Please take a moment to make sure the following forms are
completely filled out, legible and any required signatures have
been obtained.
1.
Student Information – Team Project Form (W1A). (Only one copy of
this form is required for each team.)
The team lead is responsible for filling out this form.
2.
Parent (Guardian) Signature Form (W2).(Each
student in the team is required to have a copy of their form.)
(One form is required for each team member.)
3.
Adult Sponsor Checklist for Grades 1-5 Form (W3).
(Only
one copy of this form is required for the team. The team lead
is responsible for filling out this form.)
Step
9: If the
SRC Committee requires any additional forms, please take a
moment to make sure the required additional forms are completely
filled out, legible and any required signatures have been
obtained. (You can skip this step if you answered “No” to step
2.)
Step
10: You
are almost done! Please make a copy of all your forms that will
be sent in your registration package to WSSEF. Keep the original
forms and bring them to the fair with you. Mail in only copies
of forms with your registration package. All copies must be
legible (easy to read).
Step
11: You
made it! It is time to send your registration package to WSSEF.
Please make sure you include all of the following required items
when you mail your registration package.

Entry fee of $30.00
for 2 member teams and $45.00 for 3 member teams (Checks
payable to WSSEF)

Form (W1A) (One form per team.)

form for each student in the team.)
